Please submit the following documents upon enrollment to the Registrar’s Office.

Admission credentials to be submitted:

For Senior High School graduates

  1. Original Grade 12 report card (Form 138) with the applicant’s eligibility for admission to college duly signed by the senior high school principal
  2. Original birth certificate printed on Philippine Statistics Authority (PSA) paper
  3. Original Certificate of Good Moral Character (dated not earlier than February of the graduation year) from senior high school (with school seal)

For Senior High School graduates who did not enroll in any college/university after graduation

  1. Original and photocopy of Grade 12 report card
  2. Certification from the senior high school principal regarding the student’s F137-A.
    Contents of the certification are as follows:
    1. The student’s Form 137-A is still in the school file
    2. The student’s F137-A has not been forwarded to another college/university
  3. Original birth certificate printed on PSA paper
  4. Original Certificate of Good Moral Character (dated not earlier than February of the graduation year) from senior high school (with school seal)

For foreign graduates in Philippine Schools

  1. Alien Certificate of Registration (ACR) or Special Study Permit (SSP)
  2. Original and photocopy of passport
  3. Original Grade 12 report card (Form 138) with the applicant’s eligibility for admission to college duly signed by the school principal
  4. Original Certificate of Good Moral Character (dated not earlier than February of the graduation year) from the high school (with school seal)

For applicants from schools in the Philippines or schools abroad under DepEd who did not enroll in any college/university after graduation:

  1. Original and photocopy of Grade 12 report card
  2. Certification from the high school principal regarding the student’s F137-A.
    Contents of the certification are as follows:
    1. The student’s Form 137-A is still in the school file
    2. he student’s F137-A has not been forwarded to another college/university
  3. Original birth certificate printed on PSA paper
  4. Original Certificate of Good Moral Character (dated not earlier than February of the graduation year) from the high school (with school seal)

For graduates of Non-Formal Education Accreditation and Equivalency or graduates of Alternative Learning System Accreditation and Equivalency

  1. Certificate of Rating (with passing marks in all subjects)
  2. Learner’s Permanent Record (AF-5) with eligibility to college
  3. Original birth certificate printed on PSA paper
  4. Original Certificate of Good Moral Character (dated not earlier than February of the graduation year) from the high school (with school seal)

For graduates of Associate Degree Programs

  1. Transcript of Records
  2. No failing grade
  3. Weighted average of at least 2.00 or its equivalent, excluding Religion, PE, and NSTP
  4. Honorable Dismissal/Certificate of Transfer Credential
  5. Original birth certificate printed on PSA paper
  6. Photocopy of the Certificate of Good Moral Character

For enrollees coming from schools abroad (except for Filipino schools under DepEd)

  1. Scholastic records, duly authenticated by the Philippine Foreign Service Post located in the stu-dent applicant’s country of origin or legal residence. If the country or territory of origin is already a member of the Apostille Convention, authentication of the Philippine Foreign Service is no longer necessary.
  2. Certificate of completion of secondary education (senior high school / Grade 12 or its equivalent)
  3. Certificate of Good Moral Character or Recommendation from the School Guidance Counselor or Principal
  4. For Filipino applicants:
    1. Birth certificate printed on PSA paper
    2. Original and photocopy of Philippine passport

The above-mentioned admission credentials should be submitted together with the following downloadable Mapúa forms in one (1) long brown envelope. The forms should be printed in long bond paper and should be completely filled out by both the student and the parents/guardian.

  2. Undertaking Against Joining Fraternities-Sororities
  3. Academic Grievance Undertaking
  4. Specimen Signature of Parents / Guardian
  5. Statement of Compliance
  6. Student Data Sheet (with ID picture attached/plain background)
  7. Mandatory Random Drug Testing Acknowledgement Form (MINORS)
  8. Mandatory Random Drug Testing Acknowledgement Form (STUDENTS OF LEGAL AGE)
  9. Attachment letter for Random Drug Testing
  10. RCBC MyWallet Waiver Form
  11. Malayan Insurance Form

Note: We will inform you through email confirming that we have received the above-mentioned admission credentials and the Mapúa forms. If there will be discrepancies, you will also be notified through e-mail. It is important that you comply as soon as possible so that you will be cleared for enrollment and your school IDs will be released to you upon its availability.

For Foreign nationals, please report to the Office of the Registrar, Window 5, to present your passport and ACR-ICARD so we can assist you in the processing of your visa.
